Incident report is to be completed for any incident (minor or major).   Example:  player gets a cut on his hand that is remedied via a band aid, complete the incident report.

The accident/medical report is for any incident/accident that requires treatment from a medical professional.  If this report is needed, an incident report needs to accompany it.   

All of these reports are to be completed and emailed to the Safety Director, the division director, and the league president.
